Mig wire is feeding, no puddle

I'm using a Firepower FP200 that I've run a few spools of wire through. Everything has been working great usually, but today, I can't get it to form a puddle. The wire is feeding and it tries to arc, but it's not useable. The task I was set up for was 3/8 with .035 wire.

So far I have tried:
Replacing the tip
Ensuring a shiny metal ground and clean base metal
Reducing the wire feed and voltage

Does anyone have any suggestions? It could very well be operator error but the same setup was working fine on the same metal a few days ago.
Sounds like what happens when 240v input loses one leg of power.

Is your machine auto-sensing voltage, or does it have a switch for 120/240v power that could have been bumped?

....kicking it is still an option :)
