That's a standard form. I screwed up and mentioned I weld on live gas lines. But, if you're not covered on what you're welding on and you screw up, they don't have to cover you either. They've got us in a box. I m relocating to Texas. A very small amount of red tape in that state. PA is raping me and it's about to get worse next year. F**k it. For a fraction of the $ I can get double the coverage in Texas and they don't care where you work. IMO, be as honest as you feel comfortable.
